Job Summary
Map out and oversee patient recovery processes at home by performing revisits based on skilled needs. Implement plans of care, evaluate treatment progress, and document visits electronically within 24-48 hours. Accurately observe, measure, record, and report clinical data while collecting and communicating signs of health decline. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes under RN direction. Requires NY State LPN license, one year of med/surg experience, a driver's license, and car.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Must have a driver license and car
- Good computer skills
