Job Summary
Provide nursing services and supports to individuals with brain injury, IDD, substance use disorder, or emotional/behavioral disabilities in home and community settings. Administer medications and document care; supervise and train staff in medication administration and universal precautions; assess health conditions and develop/implement health care services within each Individual Service Plan (ISP); coordinate with medical providers and external agencies to ensure consistent treatment; educate individuals served on self-medication, prescribed treatments, sex education/STD prevention, and smoking cessation. This role involves monitoring health, delivering direct care, and professional collaboration to improve independence and quality of life for the individuals served.
Required Qualifications
- Associate’s Degree in Nursing or related field, or state certificate
- One year of experience working with population served in a nursing capacity
- Current state LPN or LVN licensure for state of residence
- Current driver's license, car registration, and auto insurance
- Other certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training
