Job Summary
Provide LPN/LVN hospice nursing services under the supervision of an RN, following the Plan of Care. Observe and report patient data to the RN, implement nursing interventions, anticipate disease progression, educate patients and families about care needs, assist with understanding insurance benefits, and manage a caseload while maintaining compliance with hospice policies and Medicare Conditions of Participation. Maintain accurate documentation, collaborate with the hospice team, and apply critical thinking to address patient and family needs. Strong clinical nursing skills in hospice/palliative care, knowledge of EHR systems, and effective communication in English are required. Licensure as a Licensed Practical Nurse/LPN/LVN is required for the respective state (CA/OR/WA/TX) with related certifications and BLS within specified timelines. Proficiency in coordinating care, quality improvement activities, and adherence to professional standards is essential.
Required Qualifications
- California Licensed Vocational Nurse required upon hire
- Oregon Licensed Practical Nurse required upon hire
- Washington Licensed Practical Nurse required upon hire
- Texas Licensed Vocational Nurse required upon hire
- National Provider BLS – American Heart Association within 30 days of hire
- Infusion and Venipuncture Certification required
- Minimum 1 year LVN experience in home health or hospice (state-specific)
