LPN/LVN On-Call - Part Time

Under the direction of the On Call RN/Triage RN, supplement nursing care needs of patients with life-limiting illnesses and assist in executing the Plan of Care. Provide home visits to observe findings and report to the appropriate RN while assisting patients in learning self-care techniques. Perform specialized end-of-life care duties and ensure compliance with local, state, federal laws, Medicare regulations, and organizational policies. Submit all documentation timely and execute other required duties. Work Saturdays and Sundays from 8AM to 8PM as a part-time on-call role.
