LPN/LVN On Call - Nights

Under the direction of the On Call RN/Triage RN, supplement nursing care needs of patients as specified in the Plan of Care and provide home visits to observe and report findings. Assist the RN in executing the Plan of Care, teaching patients self-care techniques, and performing specialized end-of-life care duties. Submit all documentation timely and ensure compliance with local, state, and federal laws, Medicare regulations, and organizational policies. Work night shifts on Fridays and Saturdays with a minimum hourly rate of $35.
