LPN/LVN On-Call

Supplement the nursing care needs of patients under the direction of the On Call RN/Triage RN by providing home visits to observe and report findings. Assist the RN in caring out the Plan of Care and help patients learn appropriate self-care techniques while performing specialized end-of-life care duties. Ensure overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies. Submit all documentation timely and perform other duties as required.
