LPN/LVN On-Call

Under the direction of the On Call RN/Triage RN, supplement the nursing care needs of patients as specified in the Plan of Care and provide home visits to observe and report findings. Assist the RN in executing the Plan of Care, teaching patients self-care techniques, and performing specialized end-of-life duties while ensuring compliance with local, state, and federal laws. Submit all documentation timely and perform other required duties. Requires a current State License as a Licensed Vocational Nurse or Licensed Practical Nurse, two years of clinical experience in hospice or home health, a current driver's license, and reliable transportation with valid auto liability insurance.
