Traveling LPN/LVN provides in-home hospice nursing care in the Columbia Gorge service area under RN direction; assists with assessment data and contributes to care planning; implements the plan of care within licensure scope; promotes a team-oriented, Providence mission-driven approach; schedules are Weekdays day shift with rotating weekend coverage, requiring driving between patient homes.
Required Qualifications
- Graduate of an accredited LPN/LVN school of nursing
- Within 30 days of hire: National Provider BLS - American Heart Association
- Oregon Licensed Practical Nurse
- Washington Licensed Practical Nurse
- 1 year experience in nursing (long term care, acute care, ambulatory care or home health and hospice care)
