Job Summary
LPN/LVN Healthcare Advocate for Medicare patients, a 1099 remote contract role. You’ll guide patients through complex health concerns, build care plans addressing social determinants of health (SDOH), and act as an empathetic, action-oriented advocate to help patients achieve health goals. Requires an active LPN/LVN license, experience with diverse patient populations, strong listening and communication, clinical and organizational skills, and the ability to learn new systems quickly. This position is 100% remote and applicants must be based in the United States.
