LPN - Long-Term Care / Skilled Nursing | Travel Contract | Walsenburg, CO

Provide skilled nursing care to long-term care and skilled nursing residents, administering medications and treatments as ordered. Monitor residents for changes in condition and communicate concerns appropriately while assisting with care planning and ongoing resident assessments. Collaborate with RNs, providers, CNAs, and other members of the interdisciplinary care team, maintaining accurate and timely nursing documentation. Float within the facility as needed to support resident care. This 13-week contract runs September 8 through December 5, 2026, with 12-hour night shifts.
