LPN - Long-Term Care / Skilled Nursing | Travel Contract | Walsenburg, CO

Provide skilled nursing care to long-term care and skilled nursing residents while administering medications and treatments as ordered. Monitor residents for changes in condition, assist with care planning and ongoing assessments, and maintain accurate nursing documentation. Collaborate with RNs, providers, CNAs, and the interdisciplinary team, floating within the facility as needed to support resident care. This 13-week contract runs September 8 through December 5, 2026, with 12-hour night shifts.
