Job Summary

Provide professional nursing care for residents of all ages in long-term care under the supervision of a registered nurse or physician. Assess residents, administer medications or treatments as ordered, and perform testing to evaluate resident status. Ensure health, comfort, and safety while documenting interactions, medication outcomes, behavior, activity, and dietary needs. Counsel residents, family members, and advocates on prevention and treatment plans under direction. Recognize problems, recommend solutions, and respond effectively to emergent situations. Work as a member of an interdisciplinary care team, maintaining effective relationships with healthcare providers and the public.

Required Qualifications

  • Graduate from an accredited practical nursing program as a Licensed Practical Nurse
  • Associate degree in practical nursing
  • Must have attended a pre-licensure nursing program that included supervised clinical experience across the life span as part of the curriculum
  • Minimum of 400 hours of experience as LPN at another facility
  • Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board
  • Possess multi-state licensure privileges
  • Obtains and subsequently maintains required department specific competencies and certifications
  • Completes required annual continuing education units (CEU) and competency training requirements of the department and facility
  • Must pass a drug screen
  • Must pass pre-employment background screening

Desired Qualifications

  • Associate degree in practical nursing is preferred
