Job Summary

Provide professional nursing care for residents of all ages in long-term care under the supervision of a registered nurse or physician. Ensure the health, comfort, and safety of residents by administering medications, performing testing to evaluate resident status, and responding effectively to emergent situations. Contribute to resident assessments, document interactions and care outcomes, and counsel families on prevention and treatment plans. Establish effective working relationships within the interdisciplinary care team while maintaining confidentiality.

Required Qualifications

  • Graduate from an accredited practical nursing program as a Licensed Practical Nurse
  • Associate degree in practical nursing
  • Must have attended a pre-licensure nursing program that included supervised clinical experience across the life span as part of the curriculum
  • Minimum of 400 hours of experience as LPN at another facility
  • Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board
  • Possess multi-state licensure privileges
  • Obtains and subsequently maintains required department specific competencies and certifications
  • Completes required annual continuing education units (CEU) and competency training requirements of the department and facility
  • Must pass a drug screen
  • Must pass pre-employment background screening

Desired Qualifications

  • Associate degree in practical nursing is preferred
