Job Summary
LPN - Long Term Care/Hospice role in Northeast Georgia Health System (NGHS). Provides clinical nursing care to adult and geriatric patients in LTC and Hospice, conducts assessments, administers medications, documents care and communicates with physicians and families. Responsibilities include monitoring patient conditions, adherence to infection-control policies, ensuring safe discharge planning, coordinating admissions/transfers/discharges, maintaining supplies, and participating in unit education and staff orientation. Requires licensure as an LPN in Georgia, current BLS (or within 30 days of hire), and a state-approved LPN program graduate; minimum two years of nursing experience in adult care is expected with LTC preference. Demonstrates critical thinking, sound clinical judgment, and computer skills for unit documentation and care planning.
Required Qualifications
- Licensed as an LPN in the State of Georgia
- Current BLS certification or must be obtained within 30 days of hire
- Graduate from a state approved LPN school of nursing
- Minimum of two (2) years clinical nursing experience with emphasis on adult care in acute medical and surgical areas (required for Hospice; strongly preferred for Long Term Care)
