LPN - Long Beach NY

Conduct advanced clinical home care services including patient assessment, medication management, and patient/family education under RN direction. Perform revisits to patients based on skilled need, implement care plans, and evaluate treatment outcomes. Accurately observe, measure, record, and report clinical data while documenting visits electronically within 24-48 hours. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes. Maintain a NY State LPN License with at least one year of med/surg experience, a driver's license, and strong computer skills.
