Job Summary
Conduct patient assessments, manage medications, and perform nursing treatments such as urinary catheterization and sterile dressing changes for home care patients recovering from illness, injury, or surgery. Revisit patients based on skilled needs under the direction of a Coordinator of Care RN or an RN, implementing care plans and evaluating treatment outcomes. Accurately observe, measure, record, and report clinical data, including documenting symptoms indicating health decline, and submit electronic documentation within 24-48 hours. Reconcile and administer prescribed medications according to physician plans while providing patient and family education. Requires NY State LPN license, one year of med/surg experience, a driver's license, and good computer skills.
