Job Summary
Monitor and respond to changes in resident conditions, administer medications and treatments as prescribed, and implement physician orders accurately. Develop and maintain individualized care plans, complete assessments, and attend interdisciplinary meetings to ensure continuity of care. Lead in emergency situations by following directives and coordinating care transitions with families and external services. Maintain accurate documentation in electronic and paper formats, manage controlled substances and inventory counts, and complete incident reports. Mentor new team members, provide shift reports to nursing assistants, and foster open communication with residents and families. Participate in ongoing training and adhere strictly to infection control protocols.
Required Qualifications
- Active Practical Nursing License in Pennsylvania
- Graduate of an Accredited Nursing Program
- Temporary Practice Permit (TPP)
- CPR Certified
- Safety-First Mindset
