Job Summary
Provide professional nursing care for residents of all ages in long-term care under the supervision of a registered nurse, advanced practice provider, or physician. Evaluate resident status through testing, recognize problems, and respond effectively to emergent situations while ensuring health, comfort, and safety. Administer medications and treatments as ordered, contribute to resident assessments, and counsel residents, family members, and advocates on prevention and treatment plans. Document interactions, outcomes, and care plans related to medication, behavior, activity, and dietary needs. Establish effective working relationships with interdisciplinary teams and maintain confidentiality. Work as a member of an interdisciplinary care team to develop self-care programs and perform other assigned duties.
Required Qualifications
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse
- Associate degree in practical nursing
- Must have attended a pre-licensure nursing program that included supervised clinical experience across the life span as part of the curriculum
- Minimum of 400 hours of experience as LPN at another facility
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board
- Possess multi-state licensure privileges
- Obtains and subsequently maintains required department specific competencies and certifications
- Completes required annual continuing education units (CEU) and competency training requirements of the department and facility
