Job Summary
Provide 1:1 care for clients following their plan of care, administering medications, feedings, oxygen, and ostomy care as needed. Accurately document all nursing actions and communicate changes in medical condition to families, case managers, and physicians. Coordinate or order supplies, medications, and feedings to ensure adequate amounts are always on hand while maintaining client dignity and independence. Monday through Friday, 3:00pm to 9:00pm. G-Tube experience preferred.
Required Qualifications
- LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
- Valid driver's license
- Acceptable driving record
- Proof of car insurance
- CPR professional certification
Desired Qualifications
- 1+ years of nursing experience
- Home Health care experience
- G-Tube experience
