Administer medications, change wound dressings, and perform vital signs and treatments for residents. Develop and implement individualized care plans while conducting routine assessments, monitoring progress, and reporting changes to the healthcare team. Maintain accurate records of patient care, treatments, and outcomes. Collaborate with the interdisciplinary and rehabilitation teams to facilitate smooth transitions and ensure continuity of care. Requires current LPN licensure in Pennsylvania and ability to work full-time shifts including days, evenings, and weekends.
