Job Summary
Collaborate with the interdisciplinary team to develop individualized care plans, administer medications and treatments, and provide direct nursing care including wound care, catheterization, and vital sign monitoring. Maintain accurate medical records, educate residents and families on self-care, and adhere to infection control policies to ensure resident safety. Provide guidance to CNAs and participate in quality improvement initiatives while offering emotional support to residents and their families. Requires a valid LPN license, minimum one year of LTC experience, and willingness to work flexible schedules including weekends, evenings, and holidays.
Required Qualifications
- Valid and current Licensed Practical Nurse (LPN) license in the state of practice.
- Minimum of 1 years of experience as an LPN in long-term care or a related healthcare setting.
- In-depth knowledge of nursing principles, procedures, and practices, particularly in the long-term care context.
- Proficient in medication administration, wound care, catheterization, and other nursing procedures.
- Strong assessment and critical thinking skills to develop and implement effective care plans.
- Excellent communication and interpersonal skills to interact with residents, families, and the interdisciplinary team.
- Familiarity with electronic health record (EHR) systems and proficiency in computer literacy.
- Ability to prioritize tasks, multitask, and adapt to changing resident needs in a fast-paced environment.
- Strong organizational skills and attention to detail to ensure accurate documentation and reporting.
- Willingness to work flexible schedules, including weekends, evenings, and holidays as required.
