Job Summary
Licensed Practical Nurse (LPN) provides quality nursing care to patients as part of an interdisciplinary team. Responsibilities include monitoring and reporting changes in patient condition, assigning duties, instructing, and providing clinical supervision to CNAs to support the patient’s physical, mental, and psychosocial well-being in accordance with Life Care standards and regulations. Requires graduation from an accredited LPN program, active LPN/LVN license in the applicable state, CPR certification, and preferred one year of skilled nursing experience. Shift differentials are noted for 2nd/3rd shifts and weekends; the role emphasizes teamwork, patient care, and adherence to quality standards.
Required Qualifications

- Currently licensed/registered in applicable State; active LPN/LVN license in good standing
- CPR certification upon hire or during orientation; CPR must remain current
