Job Summary
Assess patient needs, implement registered nurse-initiated care plans, and provide direct patient care under the Nurse Practice Act. Deliver patient and family education on self-care techniques while documenting accurately and timely. Coordinate home health services by reporting identified needs to the RN and clinical supervisor for disciplines such as OT, PT, and dietetics. Prepare equipment and supplies using aseptic technique. Provide comprehensive skilled and non-skilled services to seniors, disabled, and ill clients in Colorado. Requires active Colorado LPN license, three years of nursing experience, and CPR certification. Schedule runs Monday to Friday with weekends as needed.
Required Qualifications
- Graduate of an accredited practical nurse or vocational nursing program
- Three (3) years nursing experience
- Currently licensed as a licensed practical nurse or licensed vocational nurse in the state
- Possess and maintain valid CPR certification
- Complies with accepted professional standards and practice
- Demonstrates good verbal and written communication and organizational skills
- Active Colorado LPN License
