Job Summary
Home Healthcare LPN delivers nursing care in patients' homes under RN supervision, assessing needs, implementing care plans, and educating patients and caregivers. Role emphasizes direct patient care, coordination with interdisciplinary team (HHA, OT, PT, MSW, ST, Dietitian), documentation, and adherence to plan of care. Requires state LPN/LVN license, CPR certification, and prior home health or community nursing experience; full-time benefits include health, dental, vision, PTO, and retirement plans. Work location is Colorado Springs, CO, in-person. Pay is per visit; salary is variable by visit rate, not hourly.
Required Qualifications
- Graduate of an accredited practical nurse or vocational nursing program
- Currently licensed as an LPN or LVN in the state
- Three (3) years nursing experience. Community/home health or medical/surgical experience is preferred
- Possess and maintain valid CPR certification
- Complies with accepted professional standards and practice
- Demonstrates good verbal and written communication and organizational skills
