Job Summary
LPN (Licensed Practical Nurse) for in-home medication management at Excel Home Care Services. Conduct in-home medication assessments, educate clients and caregivers on proper medication usage and adherence, set up and organize medications including pre-filling medication planners, collaborate with physicians and pharmacists, monitor for adverse reactions, maintain accurate documentation of medication administration and client progress, ensure compliance with healthcare regulations and company policies, perform health assessments including vital signs and implement care plans.
Required Qualifications
- Active LPN license in the state
- Experience in medication management or home health/geriatric care preferred
- Strong attention to detail and communication
- Ability to work independently and provide compassionate care
- Reliable transportation for home visits
