Job Summary
Provide 1:1 care for clients following their plan of care while maintaining dignity and independence. Administer medications, feedings, oxygen, and ostomy care as needed, performing required assessments of results. Accurately document nursing actions, communicate with family, case managers, and physicians regarding medical condition changes, and coordinate supplies and medications to ensure adequate availability. Ensure all patient needs are met per physician instructions. Full-time schedule with flexible options; hourly range $36.00 - $45.00.
Required Qualifications
- LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
- Valid driver's license
- Acceptable driving record
- Proof of car insurance
- CPR professional certification
Desired Qualifications
- 1+ years of nursing experience
- Home Health care experience
