LPN / Licensed Practical Nurse - Home Health

Provide 1:1 care for clients following their plan of care while maintaining their dignity and independence. Administer medications, feedings, oxygen, and ostomy care as needed, and accurately document all nursing actions, communications with family and physicians, and progress toward established goals. Coordinate or order supplies, medications, and feedings to ensure adequate amounts are always on hand, ensuring all patient needs are met per physician instructions. Full-time, M-F, 8a-5p schedule with medical, dental, vision, and 401(k) benefits.
