LPN / Licensed Practical Nurse - Home Health

Provide 1:1 care for clients following their plan of care while maintaining dignity and independence. Administer medications, feedings, oxygen, and ostomy care as needed, performing required assessments of results. Accurately document nursing actions, communicate with family, case managers, and physicians regarding changes in medical conditions, and coordinate supplies and medications to ensure adequate availability. Ensure all patient needs are met per physician instructions. Full-time schedule in Jasper, AL with coverage in Jefferson, Walker, and Winston.
