Job Summary
Provide 1:1 care to clients following their plan of care while maintaining dignity and independence. Administer medications, feedings, and oxygen; perform ostomy care as needed. Accurately document all nursing actions, communicate with families, case managers, and physicians regarding medical changes. Coordinate or order supplies, medications, and feedings to ensure adequate availability. Ensure all patient needs are met per physician instructions. Full-time schedule in Fayetteville, WV.
Required Qualifications
- LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
- Valid driver's license
- Acceptable driving record
- Proof of car insurance
- CPR professional certification
Desired Qualifications
- 1+ years of nursing experience
- Home Health care experience
