Job Summary
Provide professional nursing care for residents of all ages in long-term care under RN supervision. Perform testing to evaluate resident status, recognize problems, and respond effectively to emergent situations. Administer medications or treatments as ordered, ensure resident health and safety, and document interactions and care outcomes. Counsel residents, families, and advocates on prevention and treatment plans while maintaining confidentiality. Work as a member of an interdisciplinary care team to establish effective relationships with providers and the public.
Required Qualifications
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse
- Associate degree in practical nursing
- Must have attended a pre-licensure nursing program that included supervised clinical experience across the life span as part of the curriculum
- Minimum of 400 hours of experience as LPN at another facility
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board
- Possess multi-state licensure privileges
- Obtains and subsequently maintains required department specific competencies and certifications
- Completes required annual continuing education units (CEU) and competency training requirements of the department and facility
- Drug screen
- Pre-employment background screening

- Associate degree in practical nursing is preferred
