Job Summary
Licensed Practical Nurse (LPN) needed for full-time night shift (11pm-7am) at The Birches of Concord. Deliver nursing care to residents in a warm, home-like environment. Responsibilities include assessing, documenting, and communicating residents’ status; observing health changes and reporting to care leadership; directing and supervising Resident Care Assistants and mentoring Lead RCAs; coordinating care with community providers via case management; balancing service with resident independence; preventing elder abuse and reporting per state regulations; responding to emergencies and activating backup as needed. Requires state LPN license and CPR, graduate from approved LPN program, and 1-2 years of nursing experience (elder care preferred). Required Qualifications
- Current state LPN license
- CPR certification
- Graduate of an approved LPN program (per state requirements)
- Minimum 1-2 years of nursing experience (preferred)
