Job Summary
LPN / Licensed Practical Nurse position at Charter Senior Living (evenings and weekends) in our Assisted Living and Memory Care communities. The role focuses on resident health, wellness, and quality of life, including resident assessments, care planning support, medication and treatment management, clinical documentation, and ongoing communication with residents, families, physicians, and caregiving teams. The ideal candidate is compassionate, clinically skilled, organized, and dedicated to delivering resident-centered care in a team-oriented environment.
Required Qualifications
- Active LPN / Licensed Practical Nurse license in good standing
- Assisted living, memory care, long-term care, or senior living experience preferred
- Strong medication management and clinical assessment skills
- Excellent communication and teamwork abilities
- Compassionate and resident-centered approach to care
- Ability to work in a fast-paced environment while maintaining attention to detail
- Strong organizational and documentation skills
