Job Summary
Implement the nursing process through assessment, diagnosis, planning, implementation, and evaluation within the patient's home. Communicate with physicians and the multidisciplinary care team regarding patient health status. Provide specialized care including tracheostomy management, ventilator support, feeding tube maintenance, wound care, and seizure management. This role offers 8-12 hour shifts with a starting hourly rate of $27.00. Candidates must hold a valid Minnesota LPN license, fluency in English, and current BLS CPR certification. Physical ability to lift, turn, position, and transfer up to 50 lbs is required.
Required Qualifications
- Valid license as a Licensed Practical Nurse (LPN) in the state of Minnesota
- Ability to read, write and speak fluently in English
- Education and experience in the scope of services offered by the agency
- Basic Life Support (BLS) CPR course for healthcare providers through American Heart Association or Red Cross renewed at recommended frequencies
- Must be able to lift, turn, position, and transfer up to 50lbs
- Drug free work environment
