Job Summary
Conduct direct nursing support for older adults within the PACE program, administering medications and treatments while monitoring health status in both clinic and home settings. Document care accurately, communicate condition changes to the interdisciplinary team, and reinforce education provided by registered nurses. Prepare participants for appointments, maintain medical records, and assist with lifting and transfers to promote safety and independence. Requires current Kansas LPN license, CPR/BLS certification, and one year of nursing experience.
Required Qualifications
- Graduate of an accredited Practical Nursing (LPN) program
- Current, unrestricted Licensed Practical Nurse (LPN) license in the State of Kansas
- Current CPR/BLS certification or ability to obtain upon hire
- Valid driver's license, reliable transportation, and an acceptable driving record for travel throughout the community
- Minimum of one (1) year of nursing experience, preferably working with older adults or individuals with chronic or complex medical conditions
- Knowledge of basic nursing principles, medication administration, infection prevention, and chronic disease management
- Strong assessment, observation, and communication skills with the ability to recognize and report changes in participant condition
- Ability to work independently in participants' homes while collaborating effectively with an interdisciplinary team
- Proficiency with electronic medical records (EMR) and basic computer applications
- Ability to safely lift, transfer, and assist participants as needed
Desired Qualifications
- Two or more years of experience in home health, hospice, geriatrics, long-term care, PACE, or community-based nursing
- Experience caring for frail older adults with multiple chronic conditions
- Knowledge of Medicare, Medicaid, and PACE regulations and standards
- Experience with care coordination and interdisciplinary team collaboration
- Experience using electronic health record systems and mobile documentation tools
- Demonstrated ability to build trusting relationships with participants, caregivers, and community partners
