LPN (Licensed Practical Nurse)

Directs the daily activities of the licensed practical/vocational nurse and certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures. Follows the nursing model to promote holistic care for the residents. Participates in the admission, discharge and transfers of residents, assesses changes in residents’ status and notifies the physician and family, transcribes physician orders to the medical record and carries out orders as written. Requires a nursing degree and an active RN license, plus CPR certification. This role is in a long-term care setting and contributes to resident care, policy compliance, and collaboration with the care team.
