Job Summary
Provide professional nursing care for residents of all ages in a 79-bed long-term skilled care and transitional care facility under the supervision of a registered nurse or physician. Administer medications or treatments as ordered, perform testing to evaluate resident status, and recognize problems to respond effectively to emergent situations. Ensure the health, comfort, and safety of residents while documenting interactions, medication outcomes, behavior, activity, and dietary needs. Counsel residents, family members, and advocates on prevention and treatment plans under direction. Work various shifts including days, evenings, nights, and weekends with a requirement to complete six shifts per month.
Required Qualifications
- Must be a Licensed LPN in Minnesota
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse
- Must have attended a pre-licensure nursing program that included supervised clinical experience across the life span as part of the curriculum or have had a minimum of 400 hours of experience as LPN at another facility
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges
- Must be available to work 6 shifts per months to include two weekend shifts and holiday rotation
- Must be available for weekend shifts
- Must be available for holiday rotation
- Must be able to work various shifts (Days, Evenings and Nights)
- Must be able to work PRN shifts
- Must be able to work shifts from 6:00pm to 6:00am
- Must be able to obtain and subsequently maintain required department specific competencies and certifications
- Must be able to complete required annual continuing education units (CEU) and competency training requirements of the department and facility
- Must be able to administer medications or treatments as ordered
- Must be able to perform testing to evaluate data reflective of the resident's status
- Must be able to recognize problems, recommend solutions and respond effectively to assist in an emergent situation
- Must be able to counsel residents, family members and/or resident advocate, under the direction of a registered nurse, advanced practice provider or physician on prevention and treatment plans
- Must be able to develop a self-care program for residents
- Must be able to communicate clearly and maintain confidentiality of sensitive information
- Must be able to work effectively with professional and supportive personnel as a member of an interdisciplinary care team
- Must be able to establish and maintain effective working relationships with resident, health care providers, and the public
- Must be able to ensure the health, comfort and safety of residents
- Must be able to contribute to the assessment of residents
- Must be able to document resident interactions as needed and the outcomes related to resident care, medication, behavior, activity, and dietary
- Must be able to provide technical support to healthcare professionals as needed
