Job Summary
Conduct thorough assessments and accurately document changes in resident condition while managing medications proficiently. Promote the physical and psychosocial well-being of residents by creating a comforting atmosphere and developing meaningful relationships through learning their life stories. Collaborate with nursing and facility teams to ensure coordinated, comprehensive care that exceeds resident needs, demonstrating C.A.R.E. values of compassion, accountability, relationships, and excellence.
Required Qualifications
- Current and valid Licensed Practical Nurse license in the state of Indiana
- Proficient medication management skills
- Ability to conduct thorough assessments and accurately document changes in resident condition
- Excellent communication and interpersonal skills
