Map out and oversee the process of a patient's recovery from illness, injury, or surgery at home by providing advanced clinical home care services. Perform revisits to patients based on skilled need, implementing the plan of care and evaluating treatment outcomes. Document visits electronically within 24-48 hours and accurately record clinical data, including symptoms indicating health decline. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes under RN direction. Requires NY State LPN license, one year of med/surg experience, a driver's license, and good computer skills.
