LPN

LPN part-time role with every-other-weekend 10p-6a shifts and PRN 2p-10p, reporting to the Wellness Director. Responsibilities include medication administration, communication with resident support services and families, maintaining clinical quality assurance, and providing leadership support in resident care and services. Responsibilities also cover assessing resident needs via PSP processes, documenting changes in clinical care, assisting with wellness visits and PSP updates, monitoring weights and vital signs, and providing direct resident care as needed. Must be a licensed practical nurse with at least one year of experience with elders or disabled individuals and demonstrated leadership capabilities; experience in Assisted Living or Dementia care preferred; ability to work weekends, evenings, and flexible hours.
