Job Summary
Oversee patient recovery at home by performing revisits based on skilled need, implementing care plans, and evaluating treatment outcomes. Conduct patient assessments, manage medications, and educate families while accurately documenting visits electronically within 24-48 hours. Observe, measure, and report clinical data regarding health status, including communicating signs of decline. Administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes under RN direction. Requires NY State LPN license, one year med/surg experience, driver's license, and car.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Must have a drivers license and car
- Good computer skills
