LPN - Jersey City NJ

Conduct advanced clinical home care services including patient assessment, medication administration, and wound care under RN direction. Perform skilled patient revisits to monitor recovery from illness, injury, or surgery, and implement the plan of care while documenting visits electronically within 24-48 hours. Accurately collect, communicate, and report clinical data regarding patient health status, including signs of decline, and perform nursing treatments such as urinary catheterization and sterile dressing changes. Requires NY State LPN license, one year of med-surg experience, a driver's license, and strong computer skills.
