Job Summary
Provide in-home direct patient care according to the provider's prescribed plan of care while monitoring progress and recovery. Assess and document patient conditions including vitals and lab results, administer medications and IV fluids, and manage wound care and personal hygiene. Collaborate with providers and allied health professionals to ensure exceptional patient outcomes. Requires current CPR/AED/BLS/First Aid certification and an unrestricted WV LPN license. Flexible full-time, part-time, or PRN schedules available with no overtime required. Locations include Morgantown, Fairmont, and Wheeling.
Required Qualifications
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state of WV
Desired Qualifications
- Practical trach and/or ventilator experience
