Job Summary
Complete individualized patient care plans and perform care management services using the electronic medical record system. Maintain frequent contact with patients to coordinate care, support compliance with management programs, and document all client communications accurately. Escalate emergent patient needs to Primary Care Clinicians and keep Team Supervisors informed of care plan issues. Manage scheduling and provide support for caregivers and families while adhering to documentation protocols and best practices. Utilize secure email, telephone systems, and office equipment to meet patient and partner facility needs. Work remotely on weekends (Saturday/Sunday 8:00am–4:30pm) and weekdays (Monday–Wednesday 9:30am–6:00pm).
Required Qualifications
- Licensed Practical Nurse (LPN)
- Must be comfortable with speaking on the phone for large amounts of the day
- Must be compassionate and empathetic towards our patients, always demonstrating exceptional customer service
- Ability to take accurate notes to document each task in a timely manner
- Ability to multitask between different patients and workstreams while remaining organized and efficient with time
- Ability to thrive in a fast-paced environment
- Must be able to work from Saturday and Sunday 8:00am to 4:30pm (EST), as well as Monday, Tuesday, and Wednesday 9:30am to 6:00pm (EST)
- Must be proficient in using a computer, including Outlook and other Microsoft Office programs
- Must be able to pass a drug screen
- Must have a positive attitude
- Must be a team player
- Must be willing to learn new skills on a continuous basis
