Job Summary
Provide hands-on, in-home nursing care under RN guidance, delivering compassionate patient care, monitoring conditions, documenting progress, and collaborating with the care team to ensure high-quality hospice/home-health services. Maintain CPR certification, drive to patient homes in Colorado, and leverage clinical expertise to support patient comfort and safety. LPN/LVN license in Colorado required; 3 years' nursing experience preferred (home health or med-surg).
Required Qualifications
- Graduate of an accredited practical/vocational nursing program
- Current LPN/LVN license in the State of Colorado
- Current CPR certification
- Valid driver’s license, auto insurance, and reliable transportation
- 3 years of nursing experience preferred (Home Health or Med-Surg a plus)
