Job Summary
Licensed Practical Nurse (LPN) needed for Intensive Case Management (ICM) team in Claymont, DE. Responsibilities include developing, implementing, evaluating and revising recovery plans with participants and staff; maintaining a caseload and completing paperwork; coordinating care with community agencies and families; assessing mental health symptoms and responding to recovery plans; educating and supervising participants in self-administration of medication; ensuring annual review of physical conditions and facilitating access to treatment; local travel and potential on-call/emergency coverage; on-site new-hire orientation and training. Qualifications include DE LPN licensure or participation in NLC, knowledge of mental health and substance abuse systems, strong communication, ability to work independently, valid driver’s license, and successful background checks. Required Qualifications
- Board Certified Licensed LPN in the state of Delaware; or participating in the Nursing Licensure Compact to include the state of DE.
