Licensed Practical Nurse (LPN) sought for Horizon House Residential Group Home in Georgetown, DE. Responsibilities include assisting with development, implementation, evaluation, and revision of recovery plans; maintaining a caseload and completing paperwork; coordinating with community agencies and families; ongoing assessment of participants’ mental health symptoms and responding to recovery plans; educating and supervising participants in self-administration of medication; ensuring annual review of physical health and access to treatment; and crisis/intervention as needed. Qualifications require DE LPN license or eligibility under the Nursing Licensure Compact, knowledge of mental health and substance abuse systems, strong communication, independence, valid driver’s license, and successful clearance on Child Abuse and Adult Abuse Registry checks.
