Job Summary
Provide compassionate hospice nursing care in the home setting under the direction of a Registered Nurse. Monitor patient symptoms, assist with medication management, and document visits accurately in the electronic medical record. Educate patients and caregivers on medications, equipment, and safety while communicating condition changes to the interdisciplinary team. Assist with care planning and support the RN Case Manager to ensure patients remain comfortable and safe. This full-time Monday–Friday role includes rotating holiday coverage and double pay for holidays worked.
Required Qualifications
- Current Florida Licensed Practical Nurse (LPN) License
- Valid Florida driver's license
- Reliable transportation and current auto insurance
- Strong communication and organizational skills
- Ability to work independently while collaborating with the interdisciplinary team
- Minimum of one year of medical-surgical inpatient nursing experience
