Job Summary
Assess, plan, implement, and evaluate patient care for infants to geriatric clients in short-term home settings while partnering with families to develop teaching strategies and document education in health records. Provide professional nursing care under the direction of a Nurse Leader, utilizing the nursing process and patient-family centered principles to address daily life assistance and specialized treatment modalities. Maintain a high CMS star rating through quality care delivery across a defined territory.
Required Qualifications
- High School Diploma or equivalent
- Completion of an approved nursing program
- LPN in State of Michigan
- Driver license in State of Michigan
- BLS within 90 days
- Graduates from a non-accredited school of nursing must have at least one year of acute care experience or at least two years of experience in a licensed health care role and must have successfully passed the NCLEX licensure examination
- 1 year of relevant experience
- recent clinical experience in home health and/or other clinical setting

- Associate's Degree or equivalent
