Job Summary
Provide in-home direct patient care per the prescribed plan of care and nursing scope of practice across in-home visits; perform 1:1 patient care, skilled assessments, and develop/implement a comprehensive care plan; monitor patient progress, communicate conditions to providers, and ensure alignment with Medicare/federal guidelines; complete clinical documentation and education for patients and families; contribute to reducing readmissions through preventative care and continuity of care; require a current LPN/LVN license, minimum 2 years’ home health experience, CPR/BLS/First Aid certification, and reliable transportation; flexible scheduling with full-time/part-time/PRN options; work within Interim HealthCare’s emphasis on patient-centric care and equality of opportunity.
Required Qualifications
- Graduate of a practical nursing program (LPN/LVN)
- Current LPN/LVN license in the practicing state
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites/work locations
- Two years of experience as an LPN in home health or similar setting
- At least 1200 hours of LPN/LVN experience or completion of the office's preceptor program
- Ability to complete skilled assessments and document per Medicare/federal guidelines
- Ability to communicate patient conditions and collaborate with providers
- Ability to provide patient and family education
