Job Summary
Provide in-home direct patient care according to the prescribed plan of care and nursing scope of practice; perform skilled assessments and deliver patient-centered care in the home; develop and implement comprehensive care plans addressing patient needs; communicate patient conditions and collaborate with providers; complete clinical documentation per agency protocol and Medicare/Federal guidelines; educate patients and families; work to decrease readmissions by promoting preventative care and ensuring continuity of care.
Required Qualifications
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current unrestricted license to practice as Licensed Practical Nurse (LPN/LVN) in the state in which practicing
- At least 1200 hours of experience as an LPN/LVN or completion of the Office's preceptor program
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- Two (2) years of experience practicing as an LPN in a home health or similar setting
